I understand that Harmony Medical Concierge does not accept insurance. Upon request, I will be given a receipt that I may submit to my insurance for possible reimbursement (Harmony Medical Concierge does not guarantee insurance reimbursement). We ask that you please reschedule or cancel at least 24 hours before the beginning of your appointment. When you book your appointment, you are holding a space on our calendar that is no longer available to our other patients. To be respectful of your fellow patients and staff please contact Harmony Home Health as soon as you know you will not be able to make your appointment. A cancellation is considered late when the appointment is not canceled greater than 24 hours before the appointed time. Appointments are in high demand, and your advanced notice will allow another patient access to that appointment time. If cancellation is necessary, we require that you notify Harmony Medical Concierge at least 24 hours in advance. If an appointment is not cancelled at least 24 hours in advance you will be charged according to the scheduled service a fee to the credit card provided.
